Safety Investigation Report of the very serious accident with ro-ro passenger ship Regina Seaways, IMO 9458535, that occurred on 9 September 2025 in Klaipėda State Seaport, has been published

On 9 September 2025 a truck with a tank semi-trailer entered the main (third) deck of the ship Regina Seaways, carrying a cargo of sodium hydroxide intended for the normal operation of the ship Regina Seaways – the engine exhaust gas cleaning system. The driver parked the truck on the main deck, near the sodium hydroxide intake valve, installed on the ship. The second deck was open and the opening was situated on the left side of the truck. After completing sodium hydroxide cargo discharging to the ship’s tank, the truck driver approached the truck cabin, opened the driver's side door of the truck, catch hold of the truck’s railings and started to climb into the truck cabin. Before he finished climbing, he released hands from railings and fell down to the second deck, suffering fatal injuries. During the safety investigation it was not possible to determine the cause of the truck driver's fall to the lower deck.
